Release of Medical Records / Transferring Records

The information below will assist in you authorizing the release of your medical records (often for the purpose of transferring your records to a different physician).

Making sure your records are accurately and successfully transferred is very important to ensure the safest healthcare. When you first visit your new physician, you will fill out forms and the doctor will take a patient history.

You can help your new doctor by transferring your personal medical files from your previous physician. Your request for transferring medical records must be made in writing. Your new physician may have a form for you to use or you can download and print the form provided here.
